Patients living within the practice area are welcome to register, our reception staff can confirm your eligibility from your postcode. In this respect please bring along proof of address such as a recent utility bill. As a guide we cover the postcodes Leeds 5, 12 and 13.

You will also need to complete a registration form (GMS1) or register on- line and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team to which you should bring a specimen of urine. Suitable containers are available at reception.    If you are taking any form of medication you will need to see the Doctors prior  to requesting any  prescption.  Please book an appt to see the Health Care assistant for NEW PATIENTS CHECK .Thank you 

 If you are registering a child under six years old please bring along your red book which contains the record of immunisations.

Medical treatment is available from the date of registration.  Please contact reception for further information.

All patients should have registered named GP

From April 2015 all practices are required to provide all their patients including new patients, patients aged 75 and over with a named GP who will have overall responsibility for the care and support that they provide them.

The named GP will work with other relevant health and care professionals to ensure that patients care packages meet individual needs.

Dr H Singh has be allocated has the name GP for all patient including new patient who register from 1.4.15 onwards. HOWEVER, you can still see the GP of your choice, whenever you need to see a doctor – you don’t have to see you’re named accountable GP if you prefer not to.
